I would put your most current name down. However if your name changed after the period the tax return is for, use that one.
A simple call to HMRC will be able to give you an exact answer.

I would honestly ring HMRC tomorrow and ask them. I believe it will be your current business name but as I have never been in this situation you should definitely check it with them prior to sending it off.
HMRC have great phone service (in my experience) so you shouldn't be waiting any longer than a few minutes to speak to an advisor. They have loads of different numbers depending on your subject query so make sure you choose the right one
Can I ask you post back here when you get an answer so that if anybody else finds themselves in this situation they know what the score is?

I think you miss-understood me. I'm telling you to ring HMRC, I'm not going to do it for you
Here's the page with numbers and such on:
http://www.hmrc.gov.uk/menus/contactus.shtml
Regarding your name on other sites. There isn't a problem in you using another name as those are just usernames on websites, you should however do your accounting and finance and invoices under your organisations name, whatever that may be.

In that case, just make sure you account for both accounts in your financial reports.

Accounting via Excel, QuickBooks, or any other form of accounting software is fine. You don't even have to do it on the PC if you don't want. It is up to you, just make sure it is accurate so that when tax return time comes around you are clear on exactly what financial transactions have taken place, and when.
